The Demo Scenario
Wandoo Ridge Mine (WRM) samples its crusher crew on 12 May 2026 during a liner-change shutdown. One result changes the rest of the data set.
| Code | What it is |
|---|---|
SEG-01 |
Crusher & Screening Operators — the SEG at the centre of the story |
RA-2026-0001 |
Quantitative RCS risk assessment for SEG-01. Residual HIGH, credits spray bars + control room + PAPR |
EV-2026-0007 |
The 12 May sampling event — crusher shutdown, seven samples including a blank and a duplicate |
S-2026-0071 |
Tom Nyari's personal RCS sample. 0.062 mg/m³ — worn in a P2 disposable, not the credited PAPR |
S-2026-0072 |
Lena Vogt, same shift, same area, PAPR worn. 0.013 mg/m³. The comparison is the point |
S-2026-0077 |
Supervisor sample, QUALIFIED — 6.2% flow drift, excluded from the statistics |
S-2026-0041 |
March sample, INVALID — 12.4% flow drift. Kept for audit, excluded from every decision |
COC-2026-0031 |
Chain of custody to Terrafide Analytical Laboratories, reconciled and closed |
EA-2026-0141 |
The assessment: 248% of the shift-adjusted limit (124% unadjusted). EXCEEDANCE |
EA-2026-0139 |
The same sample assessed against the superseded 2019 limit — SUPERSEDED, kept for traceability |
EXC-2026-0011 |
The exceedance. Severity HIGH, regulator notified, work stopped, CLOSED 6 July |
INV-2026-0002 |
Investigation: two blocked spray nozzles and a procedure that permitted the wrong respirator |
CA-2026-0181 |
Replace nozzles + fit a header pressure alarm — the action that fixed the engineering control |
CA-2026-0183 |
Align the task procedure with the risk assessment on RPE — the action that fixed the paperwork |
STAT-2026-0001 |
SEG-01 group decision over the year: POORLY_CONTROLLED, exceedance probability 0.18 |
EXC-2026-0012 |
Underground DPM exceedance — deliberately left at ACTIONING, investigation still in ANALYSIS |
EXC-2026-0013 |
Post-blast CO peak of 210 ppm — CONTAINED, re-entry held, nobody exposed |
CA-2026-0191 |
Workshop LEV repair — BLOCKED on a 10-week impeller lead time. Interim PAPR hoods issued |
HSR-2026-0088 |
Silica health-surveillance referral for the affected operator. Outcome NO_RESTRICTION, nothing clinical stored |
The thread worth following: S-2026-0071 → COC-2026-0031 →
CERT-TAL-26-3391 → EA-2026-0141 → EXC-2026-0011 → INV-2026-0002 →
CA-2026-0181 → control verification EFFECTIVE → exceedance CLOSED. That is
the core chain end to end, with a real control failure in the middle.
The comparison worth making: open S-2026-0071 and S-2026-0072 side by
side. Same shift, same area, same task at 15:30. The difference is the
respirator, and the results differ by a factor of five.

Rules of Thumb
- A percentage is meaningless without its limit. Always read
percentage_of_limitnext tolimit_value_appliedandadjustment_factor. A 12-hour shift halves the limit under Brief & Scala, so 0.062 mg/m³ is 124% of the raw standard and 248% of the one that actually applies. - Invalid ≠ deleted. Sort samples by
validity_statusto see what the statistics legitimately excluded. If an invalid sample is missing from the register entirely, something has gone wrong upstream. - A control with no
ControlVerificationrow is a control nobody has checked. That is a different problem from one that failed, and the register shows both. rpe_adjusted_valueis information, never a compliance decision. It only means anything if the wearer had a current fit test on that exact respirator type — checkFitTestbefore quoting it.- Blanks and duplicates (
sample_typeBLANK/DUPLICATE) carry no exposure assessment by design. They are quality evidence, not exposure evidence. - An exceedance sitting at
VERIFICATIONis waiting on a measurement, not on an email. Look for theControlVerificationrow. SegMemberandPersonRoleAssignmentboth have end dates. Someone who left site still has exposure history, and still has a notification obligation — the demo's undelivered group-result letter is that case.
